Your personal information is collected on the page https://yogapilateswithdaphnee.co.uk/client-area. We ask clients to fill out forms in order to register with Daphnee Paquin-Katma for yoga classes and provide the relevant data which will allow Daphnee to contact you and determine if the classes will be suitable for your particular abilities and health. All data is securely stored on https://yogapilateswithdaphnee.co.uk and is never passed on to any other parties.
COVID-19 Self Assessment
The data submitted within this form is used to comply with the COVID-19 procedures set out by the U.K. government and Branksome Community Centre. The data is used purely to understand if you are a risk of transmitting COVID-19 to others, and if so attending class should be avoided.
Your basic details
The data submitted within this form is used to keep a track of clients and to provide basic contact and emergency contact details. We additionally ask about injuries that may affect your ability to perform certain yoga positions, enabling Daphnee to tailor the class to your specific abilities.
PAR-Q and you
The data submitted within this form is use to determine if you are healthy enough to practice yoga. If questions are answered with yes, Daphnee may recommend that you consult a doctor before proceeding with a yoga class.
Informed Consent
The data submitted within this form is used to satisfy insurance conditions and is intended to provide you, the client, with an opportunity to review your own health and assess whether you are capable of practising yoga with Daphnee. By submitting this form you take repsonsibility for your own prevailing health conditions.

We retain your personal data while you are a client with Daphnee. If you stop being a client with Daphee, your personal records will be removed after one month and your basic contact information will be kept on file indefinitely.

For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
